Silent Reflux Symptoms

Laryngopharyngeal reflux (LPR), commonly known as silent reflux, can cause persistent throat irritation, chronic coughing, frequent throat clearing, and changes in voice quality. Unlike traditional acid reflux, LPR may occur without noticeable heartburn, making the condition difficult for patients to recognize. As a board-certified otolaryngologist practicing in Jacksonville, I evaluate patients experiencing these problems and help determine whether reflux or another underlying condition may be responsible for their discomfort.

One of the challenges associated with silent reflux symptoms is that they often resemble those caused by allergies, chronic sinusitis, postnasal drainage, and other throat disorders. Some patients experience recurring hoarseness or a persistent sensation of something being stuck in their throat, while others develop an unexplained cough that continues for weeks or months. These symptoms may fluctuate throughout the day and can sometimes become more noticeable after meals or upon waking.

When patients visit my practice with suspected silent reflux symptoms, I carefully review their medical history, the duration and severity of their complaints, and any factors that may contribute to throat irritation. Because these symptoms alone cannot establish an LPR diagnosis, I focus on identifying their underlying cause through appropriate examination and evaluation. My goal is to help patients understand their condition and determine whether further testing or individualized treatment may be beneficial.

Silent Reflux Symptoms

Persistent Throat Clearing and Excess Mucus

Frequent throat clearing and the sensation of excessive mucus are common complaints among patients with suspected laryngopharyngeal reflux. When stomach contents reach the throat and larynx, they may irritate sensitive tissues and contribute to inflammation or a persistent feeling of mucus accumulation. I often evaluate patients who describe repeatedly clearing their throats throughout the day, even when little or no mucus is actually present. These silent reflux symptoms can become particularly bothersome during conversations, meals, or periods of prolonged speaking.

Repeated throat clearing can also create a cycle of irritation. The forceful contact between the vocal cords during clearing may aggravate already sensitive tissues, increasing the urge to clear the throat again. I explain to patients that allergies, postnasal drainage, dehydration, and certain medications can produce similar sensations. When evaluating persistent throat clearing and suspected silent reflux symptoms, I examine the throat and surrounding structures to help distinguish reflux-related irritation from other possible causes and determine whether further evaluation is appropriate.

Chronic Coughing Associated With Silent Reflux

A persistent cough that continues for several weeks without an obvious respiratory infection may be associated with laryngopharyngeal reflux. When refluxed stomach contents reach the upper throat, they may irritate sensitive tissues and stimulate the cough reflex. Some patients experience coughing after meals, when lying down, or during the night, while others develop an unexplained dry cough throughout the day. I evaluate these silent reflux symptoms carefully because coughing patterns alone cannot reliably distinguish LPR from other underlying conditions.

Chronic coughing can become particularly frustrating when it interferes with sleep, conversations, or everyday activities. I explain to patients that repeated coughing may further irritate the throat and vocal cords, potentially prolonging discomfort. However, allergies, asthma, postnasal drainage, respiratory conditions, and certain medications can also contribute to persistent coughing.

When patients seek my evaluation for chronic coughing and other suspected silent reflux symptoms, I review their medical history, examine the throat and larynx when appropriate, and consider whether additional testing is necessary. My objective is to identify the most likely cause of the cough rather than automatically attributing it to reflux. This approach helps guide treatment recommendations and reduces the likelihood of unnecessary medications or prolonged treatment that does not address the underlying problem.

Hoarseness and Changes in Voice Quality

Persistent hoarseness, a raspy voice, and vocal fatigue are among the silent reflux symptoms that may develop when refluxed stomach contents irritate the larynx and vocal cords. The delicate tissues responsible for producing sound can become inflamed or swollen, potentially affecting how smoothly the vocal cords vibrate during speech. I evaluate patients who notice their voices becoming weaker, rougher, or less consistent throughout the day. Some experience difficulty speaking for extended periods, while others report changes in their vocal range or an increased need to clear their throats before speaking.

Voice changes associated with suspected LPR can vary considerably in severity and duration. However, I emphasize that hoarseness is not specific to reflux and may result from vocal overuse, allergies, infections, benign vocal cord lesions, or other laryngeal disorders. When evaluating persistent hoarseness and other silent reflux symptoms, I may recommend examining the vocal cords with flexible laryngoscopy to identify possible abnormalities. Because hoarseness lasting longer than four weeks can indicate an underlying condition requiring medical attention, I encourage patients to seek an appropriate ENT evaluation rather than assuming reflux is responsible.

The Sensation of a Lump in Your Throat

One of the more unsettling silent reflux symptoms is the persistent sensation of having a lump, tightness, or foreign object in the throat. Medically known as globus sensation, this feeling can occur even when no physical obstruction is present. Patients often describe the sensation as something stuck in their throat that cannot be cleared by coughing or swallowing. I explain that irritation involving the throat and surrounding tissues may contribute to globus sensation, although reflux is only one of several possible causes.

Globus sensation may become more noticeable between meals, during stressful situations, or after prolonged speaking. Some patients find that swallowing food or liquids temporarily reduces the sensation, while others experience intermittent discomfort throughout the day. I carefully distinguish globus from true difficulty swallowing, which may indicate a different medical concern requiring additional evaluation.

When examining patients experiencing globus sensation and other suspected silent reflux symptoms, I consider conditions such as muscle tension, postnasal drainage, thyroid abnormalities, and structural disorders affecting the throat. Depending on the patient’s symptoms and medical history, I may examine the larynx and surrounding structures to identify possible abnormalities. My goal is to determine whether reflux may be contributing to the sensation or whether another underlying condition requires attention.

Sore Throat, Burning, and Persistent Throat Irritation

A recurring sore throat, mild burning sensation, or persistent irritation without an obvious infection may be associated with laryngopharyngeal reflux. When stomach contents reach the throat, exposure to acid and digestive enzymes can irritate sensitive tissues, potentially contributing to inflammation and discomfort. I evaluate patients who describe a raw, scratchy, or burning sensation that persists despite the absence of typical cold symptoms. These silent reflux symptoms may fluctuate throughout the day and sometimes become more noticeable after meals, during prolonged speaking, or upon waking in the morning.

Unlike the temporary discomfort associated with many respiratory infections, persistent throat irritation may continue for weeks or recur periodically. However, I explain to patients that similar symptoms can result from allergies, postnasal drainage, dry air, mouth breathing, smoking, or environmental irritants. When assessing recurring throat discomfort and other suspected silent reflux symptoms, I review the patient’s medical history and examine the throat for signs of inflammation or other abnormalities. Because irritation alone cannot confirm LPR, I use these findings to guide further evaluation and determine whether reflux or another condition may be contributing to the patient’s discomfort.

Swallowing Difficulties and Silent Reflux

Some patients experiencing suspected laryngopharyngeal reflux report discomfort when swallowing, a sensation of food moving slowly through the throat, or difficulty swallowing certain foods. These complaints may occur alongside other silent reflux symptoms, including persistent throat irritation, coughing, and hoarseness. I explain that reflux-related irritation may contribute to swallowing discomfort in some individuals, but difficulty swallowing is not specific to LPR. Problems involving the esophagus, throat muscles, or swallowing mechanism can produce similar sensations and may require a different diagnostic approach.

An important distinction I make during patient evaluations is the difference between globus sensation and dysphagia. Globus produces the feeling of a lump in the throat without necessarily interfering with swallowing, while dysphagia involves actual difficulty moving food or liquids from the mouth toward the stomach. Patients who experience food sticking, coughing or choking while eating, or progressively worsening swallowing difficulties should receive appropriate medical evaluation.

When patients report swallowing difficulties alongside suspected silent reflux symptoms, I carefully review their medical history and assess whether an examination of the throat and larynx is appropriate. Depending on their symptoms, I may recommend specialized swallowing studies or consultation with a gastroenterologist to investigate possible esophageal disorders. My priority is to identify the underlying cause rather than automatically attributing swallowing problems to reflux. I encourage prompt evaluation of persistent or worsening swallowing difficulties, particularly when accompanied by unexplained weight loss, painful swallowing, or other concerning symptoms.

When Silent Reflux Symptoms May Indicate Another Condition

Many silent reflux symptoms overlap with those caused by other medical conditions affecting the throat, nose, and upper respiratory tract. Allergies, chronic sinusitis, and postnasal drainage can produce persistent coughing, frequent throat clearing, excessive mucus sensations, and throat irritation. I also evaluate patients whose hoarseness or vocal fatigue may result from vocal cord lesions, muscle tension dysphonia, or prolonged voice use. Because these conditions can resemble laryngopharyngeal reflux, I emphasize the importance of identifying the underlying cause before recommending treatment.

Certain respiratory conditions, including asthma and chronic bronchitis, may contribute to persistent coughing, while medications such as ACE inhibitors can produce a chronic dry cough. Environmental irritants, tobacco exposure, and dry air can also aggravate sensitive throat tissues. When evaluating patients with suspected silent reflux symptoms, I consider these possibilities alongside their medical history, symptom patterns, and examination findings. My goal is to distinguish reflux-related complaints from other conditions that may require different treatments.

I also recognize that persistent hoarseness, progressive swallowing difficulties, unexplained weight loss, or a lasting neck mass can occasionally indicate more serious underlying disorders. When appropriate, I examine the throat and vocal cords using flexible laryngoscopy and recommend additional testing or specialist consultation based on my findings. By taking a comprehensive approach, I can help patients avoid unnecessary reflux treatment while ensuring that other potentially important medical conditions receive appropriate attention.

When Should You See an ENT Specialist for Silent Reflux Symptoms?

I recommend scheduling an ENT evaluation when persistent throat clearing, chronic coughing, recurring throat irritation, or unexplained voice changes interfere with daily activities or fail to improve. Although these complaints may be associated with laryngopharyngeal reflux, they can also indicate allergies, vocal cord disorders, or other medical conditions. When patients visit my Jacksonville practice with suspected silent reflux symptoms, I carefully review their medical history, assess the duration and severity of their complaints, and determine whether a detailed examination of the throat and larynx is appropriate.

Certain warning signs deserve particular attention. Hoarseness lasting longer than four weeks should prompt examination of the vocal cords, while progressive swallowing difficulties, painful swallowing, unexplained weight loss, coughing up blood, or a persistent neck mass warrant timely medical evaluation. Significant breathing difficulties or an inability to swallow require immediate medical attention. I emphasize that these symptoms should never be dismissed as ordinary reflux without an appropriate assessment.

As a board-certified otolaryngologist, I focus on identifying the underlying causes of persistent throat and voice complaints rather than assuming reflux is responsible. When evaluating silent reflux symptoms, I may use flexible laryngoscopy to examine the vocal cords and surrounding tissues or recommend additional testing when clinically indicated. My goal is to provide patients with an accurate assessment, explain their findings clearly, and recommend an individualized approach to managing their symptoms and protecting their long-term throat health.

Saw Laurie Iliff this afternoon. Excellent experience. I totally recommend her as an ENT practitioner. Made me feel very comfortable and spent as much time with me as I needed. Very, very knowledgeable. Answered all questions. I learned a lot. She is definitely a great addition to the Snowden practice.

I had an outstanding experience with Dr. Snowden and the entire office. From the moment I arrived, everyone was welcoming. Dr. Snowden took the time to listen carefully to my concerns, explained everything in a way that was easy to understand and made sure all my questions were answered before recommending a treatment plan. If you’re looking for an ENT doctor who is knowledgeable, attentive, and genuinely interested in your health, I highly recommend Dr. Snowden. I’m incredibly grateful for the excellent care I received and wouldn’t hesitate to recommend his practice to family and friends. Five stars without hesitation!

I am very happy with my experience today at Snowden ENT. I had an appointment with Laurie Iliff, APRN, and she was truly great! She listened, was thorough in her assessment, and had a very calming way about her. She communicated a clear plan for the treatment I needed to ease the symptoms I had been experiencing. This is a great office and staff!

Dr. Todd Snowden is an outstanding ENT. I recently had septoplasty and turbinate reduction with him, and the results have been incredible. My breathing has improved dramatically, and I feel better than I have in years. Dr. Snowden and his team were professional, kind, and made the entire process smooth from start to finish. Highly recommend.

I met with Nurse Practitioner, Laurie today on 4/14/26 for my ENT appointment and had an awesome visit. Laurie was so very patient, kind, reassuring, understanding, and professional. She provided the very same expertise and concerned care that I’ve grown to expect and appreciate with Snowden ENT. Laurie made me feel at ease and never rushed at any time during my appointment. She provided great insight into my ongoing treatment plan and suggested some very helpful additional options to adapt to my daily care plan. She was most informative concerning a future decision that I am presently weighing for possible Gerd Surgery after I requested her input. Her wisdom and encouragement in this area was very welcomed in order to help me make the right decision that will be best for me. So very blessed to be a patient of Dr. Snowden, Becky, and now, Laurie! Thank you all for providing the #1 ENT services in all of North Florida! With Grateful Appreciation, David L. Ashley, Patient

I’ve been a patient of Dr. Snowden since 2014, and my experience has been life-changing. After undergoing two surgeries, Dr. Snowden performed my third surgery—and it was the first truly successful one. Thanks to his expertise, I regained some of my sense of smell and experienced significant relief that I hadn’t found before. After years of struggling, I finally felt like I could breathe and function better again. I’m incredibly grateful for the care, skill, and positive results Dr. Snowden provided.
