New Patient Forms
In order to provide you with the highest level of care, we need you to provide some standard personal information, information about your current health, allergies, etc. And your medical history. We also need your signature on a few documents, and details about the medical condition that lead you to scheduling an appointment with us.
The good news is that we have made providing this information easy! Simply use your smart phone to scan the QR codes below to download either the iPhone/iPad App from the App Store or the Android device App from Google play and download our North Florida Surgeons Qure4u App. Or, open the App Store app on your iPhone/iPad or open the Google Play app on your android device and search for “North Florida Surgeons”.
Once the app is installed, click the blue login button at the bottom of the screen and when the login screen opens, click the “Sign up here” text link at the bottom of the screen and follow the instructions to create your account to access and complete the New Patient Forms.
If you do not have a smart phone or prefer to complete our New Patient Forms when you come in for your appointment, no problem. You can use our tablet to complete the New Patient Forms in the waiting area before being seen.

Your First Appointment
During your first visit, we will review your full medical history, discuss your current ENT-related symptoms or concerns, and perform a detailed physical examination. To ensure a precise diagnosis, your initial evaluation may also involve advanced in-office testing, such as fiberoptic nasal endoscopy, which allows for a closer examination of the nasal passages, vocal cords, and throat structures. In some cases, further diagnostic testing like hearing assessments, CT imaging, or allergy evaluations may also be ordered depending on your condition and treatment needs.
Following the exam and any necessary diagnostics, we will outline a personalized treatment plan tailored to your specific diagnosis. This plan may involve medications, lifestyle modifications, voice therapy, or surgical treatment when appropriate. We will take the time to thoroughly explain each option, answer your questions, and ensure that you are confident in moving forward with your treatment.

Good Faith Estimate
At Snowden ENT, we prioritize transparency in order to ensure that you have a clear understanding of the anticipated costs associated with your medical care.
As mandated by federal law, we are required to provide you with a comprehensive Good Faith Estimate that summarizes the full expected expenses for your medical services if you do not have medical coverage or choose not to use it.
- You have the right to receive a written Good Faith Estimate at least 1 business day before your scheduled or non-emergency visit. This estimate will include the projected costs of all items and services anticipated during your visit, such as medical tests, hospital fees, and prescription drugs. In the event of emergent situations or additional testing during your visit, you will receive an updated Good Faith Estimate at the time of care.
- If you prefer, you may also request a Good Faith Estimate from your healthcare provider before scheduling a service. Please note that this estimate will be based on the available information at the time of scheduling. Keep in mind that the exact cost of care can only be determined after the evaluation, as the physician may recommend or require additional services.
- It is crucial to save a copy or take a picture of your Good Faith Estimate for your records. This document will serve as a reference and provide clarity regarding the expected costs of your treatment.
- Under the new patient-provider dispute resolution, you have the right to dispute a bill if it exceeds the Good Faith Estimate by at least $400, regardless of whether you have insurance or not. We are committed to working with you to resolve any billing concerns and ensure a fair and transparent process.
